At Cypress Swamp Tattoo & Piercing Studio, safety, transparency, and compliance are extremely important to us.
To help streamline your appointment, you may download and print the appropriate consent form ahead of time. Completing your paperwork in advance helps us stay on schedule and get you in the chair faster.
Please note:
All clients must present a valid government-issued photo ID at the time of their appointment — even if forms are completed ahead of time.
Adult Tattoo Consent Form
To make your appointment as smooth as possible, please complete your consent form before your session.

Minor piercing Consent Form
For clients under 18 receiving a piercing, a notarized Minor Consent Form is required in accordance with Florida law.
Please review the requirements carefully:
• The Minor Consent Form must be printed and completed in full
• It must be signed by the parent or legal guardian
• The form must be notarized prior to your scheduled appointment time
• A valid government-issued photo ID for the parent/legal guardian is required
There is no digital option to fill out this form.
If you need a paper copy from us, please contact the studio in advance and stop by to pick one up so you have time to complete and notarize it before your appointment.
Appointments cannot proceed without a properly completed and notarized form.
